Q: how long does it stay frozen/cold?

This stays extremely cold for well over 30 minutes. I have had it on my knee for an hour and it is still cold.

Q: Do you get one or two ice pack holders?

One holder but two ice packs. Works great.

Q: How do you wrap it around your shoulders or upper neck?

It is just one single wrap, so it will wrap around the neck, but not long enough for my shoulders.

Q: Can i leave in the freezer

I have left the pack in the freezer without issue thus far.
